I-TEACH - HeartI-TEACH - Heart is a pilot project to study the impact of internet-based education and health management on people living with heart failure. The project is funded by a Heart & Stroke Foundation grant. Participants will have access to a secure portal that provides a range of tools to facilitate learning, self-management, and peer support.
